We provide timely, flexible basic support and maintenance by working in partnership with clients, participants, family, carers and other service providers.

Community Care Services aim to keep residents safe and independent in their own homes and their community. Services are offered to eligible residents who are finding it difficult to complete home tasks, activities of daily living, accessing the community and/or medical appointments, feeling isolated and have limited support networks. Services are offered across Gannawarra, Swan Hill and surrounding areas.

In the home and community-based services are offered under the following funded programs:

  • Commonwealth Home Support Program
  • Home and Community Care – Program for Younger People
  • National Disability Insurance Scheme
  • Home Care Packages
  • Veterans Home Care
  • Transport Accident Commission

Meals on Wheels

NDCH aims to deliver 20,000 meals a year through its partnership with Cohuna District Hospital and Kerang District Health and this would not be possible without the help of volunteers. Meals on Wheels deliveries take just one hour a day and make a tremendous difference to the lives of many. Meals on Wheels not only assists members of our community to remain independent and living at home, but also builds and sustains our local communities.

Social Meals Program

NDCH’s popular Social Meals program is a great way for people to socialise and enjoy a home-cooked two-course meal for just $10.

Home Care

We can help support you with house cleaning to maintain a comfortable home environment.
These tasks may include but not limited to:

  • Mopping
  • Vacuuming
  • Changing bed linen
  • Clothes washing
  • Cleaning of bathroom and toilet

Property Maintenance

We can help you with house and yard activities to maintain your home.
These tasks may include but not limited to:

  • Garden and lawn maintenance
  • Smoke detector checks
  • Light bulb replacements
  • Installation of key safes
  • Minor maintenance and repairs

Personal Care

We can help support you with personal tasks of daily life that you are unable to perform on your own.
These tasks may include but not limited to:

  • Assistance with showering
  • Dressing or grooming
  • Assistance with mobility
  • Assistance with food preparation

Food Services

This service provides a healthy, well balanced midday meal.
Meals are available:

  • 3 Courses
  • 2 Courses
  • 1 Course
  • Frozen meals

Community Meals

Our community meals consist of a 2-course home cooked meal ranging from a lovely roast to a hearty casserole.
Community Meals are held in:

  • Kerang
  • Leitchville
  • Macorna
  • Lake Charm/Mystic Park
  • Cohuna
  • Quambatook
  • Murrabit
  • Koondrook

Respite Care

Supports the carer with a break from their caring role.
Respite may be arranged:

  • On a regular planned basis
  • For a one-off emergency situation
  • For a short period where there is a change in the caring relationship eg: illness

Community Access

We can help you to access the community, social events and recreational activities.
These tasks may include but not limited to:

  • Shopping & bill paying
  • Attending appointments
  • Support with transport
  • Attending social events
  • Participate in leisure activities

Social Support

We can help keep you connected to the community, enjoying social events and attending activities of your choice.
We provide three types of social support:

  • Social support individual
  • Social support group
  • Social support transport

Gannawarra Non-Emergency Transport Service (GNETS)

The Gannawarra Non-Emergency Transport Service (GNETS) is a community transport service developed to assist people in accessing their specialist or helath care provider in larger regional areas. GNETS could not operate without our volunteer drivers. If you enjoy driving, meeting and talking to new people, and helping other in your community, consider becoming a volunteer driver with us. Your contribution can make a real difference in ensuring local residents receive the care they need.

GNETS do not travel to appointments within the Melbourne Metropolitan zone.

Do you know the NDIS Cancellation Policy?

The NDIS has updated its short notice cancellation policy as of 2022 – 2023.

Why is this important?

If you cancel a support with less than 7 days’ notice, we can claim 100% of the agreed fee from your NDIS plan. However, we will allow a 24-hour cancelation under certain conditions.
