The care will include a review of:
- medication
- mobility
- mental health
- and what matters to the person
Our chronic disease nurse helps clients manage their disease and live better lives. They work within a team, including GPs, Nursing, Allied Health and Community Care staff, that provide positive outcomes for people living with chronic disease.
If smoking or vaping is contributing to your health condition, we also offer free Smoking & Vaping Cessation Support. Our trained staff provide one-on-one, confidential help to reduce nicotine dependence and improve long-term health outcomes. This service is available across all NDCH sites and is tailored to meet your individual needs.
