Our nurses can help coordinate the care of people with chronic disease such as Chronic Obstructive Pulmonary Disease, Diabetes and heart conditions.

The care will include review of:

  • medication
  • mobility
  • mental health
  • and what matters to the person

Our chronic disease nurse helps clients manage their disease and live better lives. They work within a team, including GPs, Nursing, Allied Health and Community Care staff, that provide positive outcomes for people living with chronic disease.
