A Working Party was established to evaluate the need for community health services in Kerang, Cohuna and Pyramid Hill. A report was commissioned and conducted by Mrs. Nan Muir, assessing community health needs.

1994
Boort and Quambatook communities joined Northern District Community Health Service, enhancing service skills and offerings, as Wheatlands Community Health Service dissolved.
1996
After the closure of the Pyramid Hill Bush Nursing Hospital, Northern District begun to provide a coordinated service model in Pyramid Hill, including District Nursing, Clinic care, community education, and emergency 24-hour first aid.
2006
NDCHS begins expanding and develops to a computerized system, increasing service delivery across the region.
2010
Kerang NDCHS site was moved from Hargraves House to its first purpose-built facility on Fitzroy St.
2016
NDCHS purchased Fitzroy Street Medical Clinic from Dr Lindsay & Dr Dianne Sherriff and Dr Graeme Wood. Cohuna NDCH site is opened as second purpose-built facility.
2017
NDCHS adopts a new modern logo and officially rebrands as Northern District Community Health. NDCH begins servicing NDIS clients.
2019
NDCH celebrates 30 years of service delivery across the region.
2023
NDCH expands to include Community Care, servicing and assisting a variety of residents across the region. NDCH expands services to Swan Hill
Northern District Community Health now services four shires providing a variety of services to clients in need. We look forward to opening a new Pyramid Hill facility in the future as well as various renovations and upgrades to our current facilities.
