Rural people with chronic diseases or complex health conditions need care from a team of people with different skills. They often find it difficult to access the care they need close to home.

At the same time rural GPs often don’t have the support they need from a multi-skilled health care team to look after rural patients with complex needs.

The Sustainable Rural Healthcare Hubs trial program aims to support rural GPs in the Buloke, Loddon and Gannawarra shires to care for these patients by bringing together a network of local health services to work as a care team. Altogether it aims to contribute to good patient outcomes in rural areas.

The trial is being delivered by a health service partnership called the Integrated Health Network Alliance which includes Northern District Community Health as the lead, Murray PHN, Inglewood & Districts Health Service, Boort District Health Service and East Wimmera Health Service. The trial is being evaluated by Monash University. Funding has been provided for 4 years (2024-2027) by the Australian Government under round 4 of the Innovative Model of Care Grant program.
