The Community Health Nurse provides safe, evidence-informed and culturally responsive nursing care to clients experiencing vulnerability, disadvantage, complex health needs or barriers to accessing services in rural areas of the NDCH catchment. The role delivers clinical assessment, direct nursing care, chronic condition management, preventive and wellbeing support, post-acute and palliative care support, and coordinated referrals through NDCH sites, outreach and home visits.
The position is an accountable, clinically autonomous Registered Nurse role within its authorised scope of practice. It also coordinates the client service pathway, including triage, program eligibility verification, prioritisation, client allocation, waitlist management, clinical follow-up, documentation quality and accurate funded-service reporting. The nurse works collaboratively with NDCH’s multidisciplinary workforce and escalates matters beyond their clinical authority or available service capacity to the Nursing Manager or appropriate clinician.
